Insurances
Do to the ever changing insurance environment, we cannot guarantee that we participate in your insurance plan(s). Please contact your insurance company to verify our participation. Below is a rough guide that we do try to keep up to date as we learn more.
Government Plans

PPO / EPO / POS Plans
- Aetna (We are not in some EPO plans so check with Aetna)
- Anthem Blue Cross (We accept "Pathway X - Tiered" and "Pathway Tiered" EPO plans in Exchange Region 18)
- Blue Shield (We do NOT accept Covered CA plans. We do NOT accept plans in group X0001000. We do NOT accept plans with id prefixes: XED, XEK, XET, XEW, XES, XEF.)
- Cigna (We accept Cigna + Oscar plans)
- Health Net (We do accept PureCare One EPO plans and PureCare Health Care Service Plans (HSP))
- Humana
- Interplan
- UnitedHealthcare (We are out-of-network for UHC Group Medicare Advantage PPO plans)
Billing
After your visit is billed and processed by your insurance company, you may owe a balance. Please make prompt payment as soon as you receive your statement. We do offer payment plans if you cannot afford to pay the balance in full. If your account has a balance for a period of time in which no payment has been made, it will be forwarded to our collection agency. Beware that you may be discharged from our practice if your account remains in collections.
We provide convenience ways to pay your balance.
Pay Online
Visit the patient portal to securely view your account details and pay on-line. There you can also make a one-time payment using information found on your statement without an account.
Pay By Mail
Mail your bill payment stub along with your check to the payment address below. Make checks payable to "Dr. Joan Sy Medical Corporation."
