Contact the OfficeWe participate in many Aetna EPO and PPO plans, including Elect Choice EPO, Open Choice PPO, and Managed Choice POS. Some Aetna EPO plans are out of network; please verify your specific plan. We are also out of network for Aetna Medicare Signature (PPO) plans.
We are out of network for some narrow or Select EPO/PPO plans, including plans such as California Choice and Prudent Buyer Exclusive. We are in network for standard Anthem PPO plans, including Prudent Buyer and Anthem High Performance.
We are out of network for Covered California plans. We are also out of network for plans in group X0001000 and plans with ID prefixes XED, XEK, XET, XEW, XES, and XEF. We are in network for BlueHPN and the Tandem PPO Network as of January 1, 2025.

We are not accepting new UnitedHealthcare patients at this time. We are also out of network for UnitedHealthcare Group Medicare Advantage PPO plans.
We accept many out-of-network plans, but out-of-pocket costs may be higher depending on your plan benefits.
After your visit is processed by your insurance company, you may owe a balance. Please make payment promptly after receiving your statement. Payment plans are available for eligible balances.
If no payments are made over an extended period, balances may be forwarded to a collection agency and ongoing care may be affected.
