Insurances

Insurance Information

Because insurance plans and provider networks change frequently, we cannot guarantee participation in every plan offered by an insurance company. We strongly encourage patients to contact their insurance provider directly to confirm whether our office is in network for their specific plan.

The information below is provided as a general guide and is updated as we receive new information. Coverage may vary by employer group, plan type, and effective date.

We do accept many out-of-network plans; however, patients may have higher out-of-pocket costs depending on their plan’s out-of-network benefits.


Government Plans

  • Medicare
  • TRICARE West
  • CHAMPVA
  • Veterans Affairs Patient-Centered Community Care (VAPC3)

HMO Plans

  • Monarch Medical Group
    Panel closed to new patients

PPO / EPO / POS Plans

  • Aetna
    We participate in many Aetna EPO & PPO plans, including Elect Choice EPO, Open Choice PPO, and Managed Choice POS. Some Aetna EPO plans are out of network. Please verify your specific plan with Aetna.
  • Anthem Blue Cross
    We are out of network for some narrow or Select EPO/PPO plans, including plans such as California Choice and Prudent Buyer Exclusive. We are in network for standard Anthem PPO plans, including Prudent Buyer and Anthem High Performance.
  • Blue Shield of California
    We are out of network for Covered California plans. We are also out of network for plans in group X0001000 and plans with ID prefixes: XED, XEK, XET, XEW, XES, and XEF. We are in network for BlueHPN and the Tandem PPO Network as of January 1, 2025.
  • Cigna
    We participate in many Cigna PPO plans. Coverage varies by plan, so please verify with Cigna.
  • Health Net
    We are out of network for Covered California plans, including Ambetter PPO. There may be additional exclusions. Please verify your plan with Health Net.
  • Humana
    We participate in select Humana PPO plans. Please verify your coverage.
  • Interplan
    We participate in Interplan PPO. Coverage depends on your employer group and plan details.
  • UnitedHealthcare
    We are not accepting new UnitedHealthcare patients at this time. We are also out of network for UnitedHealthcare Group Medicare Advantage PPO plans.

Billing

Billing

After your visit is billed and processed by your insurance company, you may owe a balance. Please make prompt payment as soon as you receive your statement. We do offer payment plans if you cannot afford to pay the balance in full. If your account has a balance for a period of time in which no payment has been made, it will be forwarded to our collection agency. Beware that you may be discharged from our practice if your account remains in collections.

We provide convenience ways to pay your balance.

Pay Online

Visit the patient portal to securely view your account details and pay on-line. There you can also make a one-time payment using information found on your statement without an account.

Pay By Mail

Mail your bill payment stub along with your check to the payment address below. Make checks payable to "Dr. Joan Sy Medical Corporation."
